Paraneoplastic Syndromes Flashcards
1
Q
Prevelance of paraneoplastic sydrome with lung cancer
A
~ 2%
2
Q
9 cateogires of lung cancer related paraneoplastic syndrome
A
- Hypercalcemia
- SIADH
- Cushing’s syndrome
- Bone changes
- Neuropathy
- Myopathy
- Anemia
- Hypercoaguability
- Dermatologic changes
3
Q
Characteristics of PNS (hypercalcemia)
A
- Metastatic bone lesions
- Elevated PTH (SCC)
- Incidence: 10% of lung cancer patients
- Sx: constipatin, anorexia, polydipsia, irritability
4
Q
Characteristics of PNS (SIADH)
A
- Associated with SCLC and women
- hyponatremia with elated UNa
- nausea, seizures, AMS
- Tx: fluid restriction +/- demeclocycline (AHD inhibion in kidney)
5
Q
Characteristics of PNS (Cushing’s syndrome)
A
- Associated with SCLC
- Autononmous ATCH production
- No suppression with dexamethasone
- Sx: edema and muscle weakness
6
Q
Characteristics of PNS (bone changes)
A
- Clubbing (SCC)
- Hypertrophic pulmonary osteoarthropathy
- radius, ulna, tibea, fibula
- new bone formation and inflammation at edges of bone > pain
- tx: NSAIDS
7
Q
Characteristics of PNS (neuropathy)
A
- common and multifactorial
- brain or neurologic mets vs. circulating antibodies
- neuropathies frequently preceed lung ca diagnosis
8
Q
Characteristics of PNS (myopathy)
A
- Proximal muscle weakness
- Etiologic theories:
- Weakness manifestation of myosistis
- Defect in neuromuscular transmission
- Eaton-Lambert syndrome
- Assoc with SCLC
- Ig against voltage-gated Ca channels (inhibits presynaptic Ach release)
- Etiologic theories:
9
Q
Eaton-Lambert syndrome
A
- Assoc with SCLC
- Ig against voltage-gated Ca channels (inhibits presynaptic Ach release)
10
Q
Characteristics of PNS (anemia)
A
- Common in lung ca
- Multifactorial cause:
- decreased RBC life span
- decreased Fe stores
- decreased Fe binding capacity
11
Q
Characteristics of PNS (hypercoaguability)
A
- Common in lung cancer
- Associated with vascular thrombosis
- Maybe resistent to anticoagulation
- Treatment of tumor may reverse hypercoaguble state
12
Q
Dermatologic PNS manifestation associated with lung cancer
A
- Dermatomyositis
- Erythema gyratum
- Lanunioa acquisita
13
Q
Dermatologic PNS manifestiations MC assocated with what lung cancer histologic subtype
A
Adenocarcinoma
