Normal
Screen in 3 years age 21-29, age 30+ can do every five years if co-testing for HPV - if HPV positive but PAP normal repeat in 1 year
ASC-US
Age 21-29, repeat PAP in 1 year with reflex HPV test
Age 30+ if HPV negative repeat co-test in 3 years, if HPV positive, refer to Colposcopy
LSIL
Age 21-24, repeat in one year
Age 25-29, refer to colposcopy
Age 30+, if HPV negative then repeat PAP in 1 year (may colposcopy), if HPV positive refer to Colposcopy
ASC-H
Refer to colposcopy all ages
HSIL
Refer to colposcopy, if 25+ then consider immediate excision treatment as well
AGC
Follow up testing based on specific findings
Results have a ‘H’
Refer to colposcopy
Anything other than normal in HPV positive and 30+ years old
Colposcopy
What is ASC-US?
atypical squamous cells of undetermined origin. Always a repeat test in 1 year (or 3 co-test) unless 30+ and HPV positive (refer to colposcopy)
What is LSIL?
Low grade squamous intraepithelial lesion. Refer age 25+ to colposcopy, if younger just repeat in 1 year
What is HSIL?
High grade squamous intraepithelial lesion. Most significant, always a colposcopy refer or immediate excision
What is ASC-US?
Atypical squamous cells of undetermined significance. In 29 or younger, do HPV reflex and repeat PAP in 1 year. If 30+, repeat co-testing in 3 years unless also HPV positive (refer to colposcopy)
What is ASC-H?
Atypical squamous cells cannot rule out H. Refer all ages to Colposcopy to rule out HSIL
What can affect results?
sexual intercourse, douching, baths, tampons, menstruation, and so forth.
First, ask the patient whether any of the mentioned activities occurred within 48 hours of the test, if so consider re-schedule
PAP results are ‘Unsatisfactory’
Repeat test in 3 months
