Pancreatic carcinoma typical pt
> 70 year old male
Pancreatic carcinoma pathology
Mostly ductal adenocarcinoma (metastasize early, present late)
60% arise in head, 25% body, 15% tail
Pancreatic carcinoma presentation
Painless obstructive jaundice: jaundice+palpable gallbladder
Anorexia, Weight loss
May present like acute pancreatitis
Hepatomegaly, splenomegaly, lymphadenopathy, ascites
Pancreatic carcinoma tests
Increased CA19-9 in blood
US/CT may show mass
ERCP/MRCP identifies mass location
Pancreatic carcinoma treatment
Resection (pancreaticoduodenectomy, Whipple’s) if no metastases
Laparoscopic excision if tail lesion
Stent insertion for palliatives care
Opiates/coeliac plexus infiltration with alcohol for pain
Pancreatic carcinoma prognosis
Mean survival <6 months
5yr survival 3%, 5-14% with Whipple’s
