PALS BRADY
Epinephrine .
0.01mg/kg IV/IO (0.1mL/kg of 1:10,000) q3-5mins (Tip 1cc of 1:10,000 epi for every 10kg)
PALS BRADY
Atropine
0.02mg/kg/dose IV/IO; may repeat x1 (min dose 0.1mg, max single dose 0.5mg)
* atropine no longer primary. Use if increased vagal tone or AV block.
PALS PULSELESS OR UNSTABLE VT/VF:
Defibrillate biphasic
2J/kg initially, then 4J/kg, up to max 10J/kg or adult dose
PALS PULSELESS OR UNSTABLE VT/VF:
Epinephrine
0.01mg/kg IV/IO (0.1mL/kg of 1:10,000) q3-5mins (Tip 1cc of 1:10,000 epi for every 10kg)

Amiodarone
5mg/kg IV/IO may repeat x3

MgSO4
25-50 mg/kg/dose (max 2g)
PALS SVT (WPW, AVNRT, AVRT) Adenosine
0.1mg/kg rapid push, 2nd dose 0.2mg/kg
max 2nd dose 12mg
PALS SVT (WPW, AVNRT, AVRT) Cardioversion
0.5-1 J/kg then 2J/kg
PALS SVT (WPW, AVNRT, AVRT) Amio OR Procainamide (below)
Amiodarone 5mg/kg IV/IO over 20-60mins OR Procainamide 15mg/kg over 30-60mins
do not routinely give procainamide/amiodarone together
PALS STABLE VT w PULSES
Cardioversion
0.5-1 J/kg then 2J/kg
PALS STABLE VT w PULSES
Adenosine if rhythm regular and QRS monomorphic
0.1mg/kg rapid push, 2nd dose 0.2mg/kg
max 2nd dose 12mg
PALS STABLE VT w PULSES
Amiodarone OR Procainamide
Amiodarone 5mg/kg IV/IO over 20-60mins OR Procainamide 15mg/kg over 30-60minsdo not routinely give procainamide/amiodarone together
