H’s & T’s
H's: - hypo-XIA - hypo-volemia - hypo-thermia - hypo-/hyper-kalemia - hypo-glycemia - H+ ions (acidosis) T's: - trauma - toxins - tension pnuemo - thrombosis, pulm - thrombosis, coronary
PEDI: Epi dose
0.01 mg/kg (0.1 mL/kg of 1:10,000), repeat q3-5 min
PEDI: Amiodarone dose
5 mg/kg bolus, may repeat up to 2X for refractory VF or pulseless VT
PEDI: Defib energy dose
First: 2 J/kg
Second: 4 J/kg
Subsequent: max 10 J/kg (up to max adult dose, 120-200 J biphasic or 360 J monophasic)
PEDI: HR cut-offs for sinus tach vs. SVT
220 bpm for infants; 180 for kids (n.b. there is NO beat-to-beat variability with SVT while this is normal in sinus tach)
PEDI: Synchronized cardioversion energy dose
0.5-1 J/kg —> (1 J/kg) —> 2 K/kg
PEDI: Adenosine dose
1st: 0.1 mg/kg (max 6 mg) as RAPID bolus
2nd: 0.2 mg/kg (max 12 mg)
PEDI: Lidocaine dose
1 mg/kg loading dose + 20-50 mcg/kg/min maintenance drip
PEDI: Mag sulfate dose
25 to 50 mg/kg IV/IO (max 2 g) given over 10-20 mins [faster if TdP arrest]
DOPE Mnemonic for acute decompensation of intubated patient
D - displacement of the tube (hemi-intubation)
O - obstruction of the tube (kink, secretions)
P - pneumothorax
E - equipment
