Atropine dose
First 0.5 mg, repeat q3-5min (max 3 mg)
Epinephrine (for arrest)
1 mg IV/IO q3-5 min
Vasopressin (for arrest)
Dose of 40 units can replace first two epi doses
Amiodarone (for arrest)
[FOR VT/VF]: 150 mg over 10 minutes; repeat PRN for recurrence and start maint. infusion of 1 mg/min for 6 hours
Dopamine drip
5-10 mcg/kg/min (in 70 kg adult = 350-700 mcg/min)
Epinephrine drip
0.1-0.5 mcg/kg/min (in 70 kg adult = 7-35 mcg/min)
Adenosine (for unstable VT or reg, monomorphic VF)
First 6 mg rapid IV push, Second 12 mg rapid IV push
Mag sulfate
FOR TdP (or arrest in alcoholic, malnourished or known/suspected hypomag. patient): LD bolus of 1-2 g IV/IO in 10 mL D5W over 5-20 minutes
Lidocaine
FIRST: 1 to 1.5 mg/kg IV/IO
THEN: 0.5-0.75 mg/kg q5-10 min (MAX 3 mg/kg total doses)
Norepinephrine drip
0.1-0.5 msg/kg/min (in 70 kg adult = 7-35 mcg/min)
Hs and Ts
Trauma Tamponade Tension PTX Thrombosis, pulm Thrombosis, coronary
Hypoxia Hypovolemia Hypo-/hyperkalemia Hypothermia Hypoxia Hypoglycemia (Pedi)
Target temp and time for therapeutic hypothermia
32-34°C for 12-24 hours (still ok to perform PCI while hypothermic)
Bolus fluid temp for pt’s receiving therapeutic hypotension
