Medications that trigger migraines
Ttt of choice for moderate to severe migraine and when analgesic therapy failed
Triptans (sumatriptan 50mg)
With or without aura
- Buclizine
- prochlorperazine
- metoclopramide
N&V accompanying migraines
- TOC for headache of children younger than 12 years old
- SE: liver toxicity (high dose)
- interaction: warfarin
Acetaminophen
Combination therapy for migraine
Analgesic + caffeine:
(paracetamol, aspirin, excedrin)
Interaction: mao, ergot, st johnson’s wort
CI: not prophylactically, cardiovascular disease, cerebrovascular disease
Sumatriptan
Most serious complication of poorly managed pain
Disability and loss of function
In the US, they are approved for prescription use only (POM)
Topical NSAIDS
• ibuprofen • ketoprofen • felbinac • piroxicam Precautions: Asthmatic patients
- Topical NSAIDS: gels, creams and ointments
- same benefits as systemic NSAIDS, but less SE.
- pain relief through nerve stimulation rather than depression.
- minor aches of joints and muscles
- used as adjuncts
- paradoxical pain relieving effect: create less severe pain to counteract a more intense one
Counterirritants
- Acetaminophen
- Methyl salicylate
- CAM glucosamine
What do they have in common ?
