Adult Dilaudid dose
0.5-1mg IV, 1-2mg IM

How to administer pediatric Dilaudid
Adult fentanyl dose
1-2mcg/kg (50-150mcg) IV/IM/IN
Pediatric fentanyl dose
1-2 mcg/kg
Fentanyl infusion dosing Adult
200mcg/100ml OR 500mcg/250ml
After initial bolus: 1mcg/kg/hr.
Then bolus 1 MCG/KG and increase in fusion by 1 MCG/KG/HR. Max bolus 100, max infusion 3 MCG/KG/HR (200 MCG)
Fentanyl infusion, dosing, pediatric
0.5 mcg/kg/hr after initial IV bolus After 5 – 10 minutes bolus 0.5 mcg/kg and increase infusion by 0.5 mcg/kg/hr. Repeat bolus and infusion titration q 5 – 10 min PRN to a max of 3 mcg/kg/hr.
Adult morphine dosing
2 – 5 mg IV; 5 – 10 mg IM.
Q15min PRN
Pediatric, morphine dosing
0.1 – 0.2 mg/kg IV/IM
Q15Min PRN
Ketamine dose for pain medication, adjunct
Adult and pediatric:
Bolus dose: 0.2 mg/kg slow IV push
May repeat q 30 minutes PRN
Adult Ativan dose for sedation
1-4 mg Q45min
Pediatric Ativan for sedation
0.05-0.1 mg/kg IV OR IM
Adult Versed for sedation
2–5mgIVorIM
Intubated patients: approximately 0.05 mg/kg – max dose 5 mg.
Non-intubated patients: Use lower end of dosage range unless patient condition dictates higher doses. Use with caution in frail, elderly or debilitated patients - consider starting dose of 1 mg

Pediatric Versed for sedation
0.05 – 0.1 mg/kg IV or IM.
0.3 mg/kg IN (max dose 10 mg)
Adult Versed drip
10mg in 100ml OR 20mg in 100ml
<100kg: 2mg/hr after bolus
>100kg: 4mg/hr after bolus
If sedation inadequate after 5 – 10 minutes, bolus 2 mg and increase infusion by 1 mg/hr. Repeat bolus and increase infusion q 5 – 10 min PRN, not to exceed an infusion rate of 10 mg/hr
