treat pts with chronic pain with
- long acting opiates
- high dose, short acting opiates are more likely to lead to tolerance
starting patients on chronic opiate therapy? make sure to get a
opiate agreement
need to order continuous on any patient on PCA (patient controlled analgesia)
pulse oximetry
if discontuing PCA (patient controlled analgesia), discontinue the
- basal rate
- leave demand dose only
use PCA (patient controlled analgesia) for
- short term only
avoid these pain medications in ESRD
- morphine
- meperidine
- vicodin
- codeine
drug of choice for ESRD
- fentanyl
- methadone
avoid for treatment of pain in patients with hepatic disease
