Pagets disease and bone malignancy Flashcards
Who are most likely to get pagets disease of bone
Over 40s, 1st degree relatives, predisposed with viral environment
Describe the pathophysiology of pagets disease of bone
increased osteoclastic resoption accompanied with increased osteoblastic activity.
Normal bone replaced with abnormal haphazard bone, marrow replaced with fibrous tissue and blood vessels
How can pagets disease of bone present
Bone pain of long bones, skull, pelvis
Degenerative joint disease, fractures deformity
Neurological syndromes like deafness, spinal claudication
How will PDBpresent upon examination
Deformity, tenderness, warmth
How will PDB bone present on plain radiographs
Bones typically expanded, cortical thickening, coarsened trabeculae, lytic and sclerotic areas
What are some complications of PDB
Deformity, joint pain, fracture, osteosarcoma
How is PDB treated?
Bisphosphonates inhibit bone resorption reducing symptoms (Adcal)
Asymptomatic to be observed only
Analgesia if pain, treatment begins
What is the definition of a tumour/neoplasm
Cells keep dividing when new cells are not required, casuing a mass of tissue (unregulated clonal irreveersible clonal division)
Describe osteoid osteoma
A painful, self-limiting bone lesion caused by osteoblasts
What is osteochondroma
Cancer of the cartilage
What is a cancer of fibrous tissue
fibroma
What is a haemangioma
vascular cancer
What is the primary malignant cancer of bone
Osteosarcoma
under 20s, long bones, growth plates
What is the primary malignant cancer of cartilage
Chondrosarcoma
40s, painful, progressive
Name two forms of primary malignant bone marrrow cancner
ewings sarcoma (childhood, medullary cavity), myeloma
