What are the aims of paediatric dentistry?
an intact permanent dentition
* no active caries
* few restored teeth as possible
* a positive attitude to their future dental care.
* restorations should be to the highest standard possible to maximise the longevity of the restorations
What are the operative differences between children and adults?
*Developmental maturity/ behaviour etc.
*Constant change
*Developing dentition
*Operator access
*Tooth size and shape
*Choice of restoration
What is the sequence of paediatric restorations?
- prevention
- fissure sealants
- preventive restorations
- simple fillings e.g.. shallow cervical cavities
- fillings requiring LA but not into pulp*
- pulpotomies**
- In co-operative children ** upper arch first
How should minimal cavities be treated?
hand excavation or limited caries removal with a slow speed handpiece may not require LA
How should all other cavities be managed?
- all other cavity preparation DOES require LA
- use topical anaesthetic
What is the lignocaine max dose?
Lignocaine max dose is 4.4mg/kg. (A 10th of a cartridge per kg).
How much active agent does a 2.2ml cartridge of lignocaine contain?
44mg
What is prilocaine 3% max dose?
max dose is 6mg/kg. (Just less than a 10th of a cartridge per kg).
How much active agent does a 2.2ml cartridge of prilocaine have?
66mg
What is the morphology of the primary central incisor?
Root may bend towards the distal.
Mesial edge of crown tends to be straighter
What is the primary canine morphology?
Again mesial edge tends to be straighter with distal flaring out soon after the ACJ
What is the morphology of the upper 2nd primary molar?
prominent transverse ridge
two buccal roots
one palatal root
What is the morphology of the lower 2nd primary molar?
three buccal cusps similar to permanent first molar
What is the morphology of the upper 1st primary molar?
prominent mesiobuccal
What should the cavity design be in primary molars be?
- occlusal portion no greater than 1.5mm
- include all pits and fissures but preserve transverse ridges unless undermined by caries.
