What is a common mimic for thoracic pathology in kids <5yo?
The thymus
- can be very large at times post infection, potentially mimicking a large pneumonia taking up an entire upper lobe (particularly on the Right upper lobe)
- Becomes much small after 5yo
- Ultrasound can differentiate from other structures
How useful is not seeing the appendix in a paedaitric abdominal ultrasound?
Not visualising the appendix of oedema in the R)IF has a 90% NPV for appendicitis
How useful is ultrasound for ruling out paediatric ovarian torsion?
Bilateral normal sized symmetrical ovaries has an NPV of 90%
Normal doppler of the ovary is not reassuring if there are other signs of torsion
How may a radiolucent (the majority) inhaled foreign body in the lung otherwise be detected?
It will often act as a one way valve leading to hyper-expansion of the affected side, most notable with an expiratory film

- Worsening prognosis with higher grade, high grade at risk of growth arrest from physeal failure
- Normal fracture complications (nerve injury, vascular injury, compartment syndrome
- Complex regional pain syndrome aka reflexsympathetic dystrophy
