Mx of cow’s milk protein allergy/ intolerance in formula fed children?
1st line: extensive hydrolysed formula (eHF) milk
if no reponse/ severe CMPA: Amino acid-based formula
Mx of cows milk protein allergy/ intolerance in breastfed infants?
continue breastfeeding, eliminate cows milk protein from maternal diet. use eHF milk when breastfeeding stops
What is used to reintroduce milk protein in children (> 6 mo) with cows milk protein allergy?
Milk ladder - first introduces cooked/ baked milk. children start with malted milk biscuits and work towards pasteurised milk
Red flags for immediate referral to hospital in bronchiolitis?
- apnoea
- looks seriously unwell
- severe respiratory distress (grunting, marked chest recession, RR>70)
- central cyanosis
- persistent sats <92% RA
Mx of bronchiolitis?
Humidified oxygen via head box if sats <92%.
- NG feeding if inadequate oral intake
- suction if excessive upper airway secretions
What is Palivizumab?
Palivizumab is a monoclonal antibody which is used to prevent RSV in children who are at increased risk of severe disease.
- e.g. premature infants, lung/ heart abnormalities, immunocompromised
School exclusion rules for impetigo?
should be excluded from school until all lesions are crusted and healed or 48 hours after commencing antibiotics
Mx of extensive impetigo?
oral flucloxacillin (erythro if pen allergic)
Mx of limited localised impetigo?
1st line: 1% hydrogen peroxide cream.
2nd line: topical fusidic acid (if MSRA, topical mupirocin)
Suspected meningococcal septicaemia, what tx to give kids <1 yr old?
IM Benzylpenicillin 300mg

600mg IM Benzylpenicillin
Suspected meningococcal septicaemia, what tx to give if kid >10 years old?
1.2g IM Benzylpenicillin
