Paediatric Trauma III Flashcards

1
Q

what is a trauma stamp and when is it used?

A

A list of things that can be used to check a tooth with trauma:
- Sinus
- Colour
- Mobility
- Tender to percussion (TTP)
- Percussion
- Ethyl Chloride
- Electric pulp test (EPT)
- Radiograph

used during trauma incident to give better idea of the state of the trauma’d tooth

Q

what is an extrusion and how would you treat it?

A

a trauma accident where the tooth is displaced out of the socket partially

LA - gently reposition tooth back into socket then splint

Q

what is lateral luxation and how would you treat it?

A

Displacement of a tooth in socket laterally, can usually involve alveolar fracture

splint - 4 weeks!

LA - gently reposition and splint. Evaluate endodontic treatment 2 weeks post injury.

If open apex, there may be revascularisation, if its closed, most likely necrotic pulp and endo needed.

Q

what is intrusion and how would you treat it?

A

tooth forced into socket axially and locked into bone.

If very intruded, surgical reposition may be required 3-7+mm
less than 3mm can be used with splint under LA or orthodontic elastic bands.

Open apex - 40% survival rate pulp
Closed apex - 0% survival rate - endo

Q

what is a tooth avulsion and how would u treat it

A

tooth totally displaced from socket. [emergency situation]

Critical factors: storage medium of tooth out of mouth, saliva/milk etc. If PDL is damaged or tooth has dried out. If tooth out of mouth for 60mins+ = PDL cells are NON-VIABLE

Reimplant under LA - Splint.

Closed Apex = endo
Open apex = possible endo (if spontaneous healing doesnt occur)

Q

what is a dento-alveolar fracture and its clinical findings?

A

fracture of alveolar bone which may or may not involve the alveolar socket

gingival laceration, occlusal disturbance, segments of the mouth are mobile and displaced with several teeth moving together

Q

how do you treat a dentoalveolar fracture?

A

reposition any displaced segment of bone. Stabilise by splint, suture any soft tissue lacerations. Monitor pulp of teeth involved

[soft diet, good oh etc]
