a baby presents with loud machinery murmur below the left clavicle radiating to the back with poor feeding, tachycardia, and tachypnea and hepatomegaly. When you feel their pulses they are bounding. they also have no red reflex.

congenital rubella syndrome

this description is characteristic of PDA. this leads to oxygenated blood returning to the lungs again and again from the aorta. this reduces peripheral oxygenation and also leads to pulmonary hypertension –> oedema and right sided heart failure. Poor feeding occurs either if the blood vessels and the lungs have to work much harder to handle the extra blood. This can lead to fluid buildup in the lungs. The baby can have a harder time breathing and feeding or if the pulmonary pressure increases beyond the systemic pressure allowin blood to flow from right to left across the defect, causing cyanosis. This is called Eisenmenger syndrome. peripheral pulses due to increased cardiac output
