What are PAC’s considered?
An atrial dysrhythmia
Is there a P wave?
Yes; its premature and abnormally shaped or hidden
How does a PAC occur?
An ectopic focus in the atria fires before the next SA node impulse is due. This causeses atrial depolarization. Its premature BC it happens before the next expected heart beat.
Is there a QRS?
YES;
Is there a PR interval
Yes; its normal and <.20 seconds
Is the rate regular?
No!
Is the rhythm regular?
NO! BC of the premature beat the rhythm is irregular.
What’s the heart doing during a PAC?
Another cell is firing and wants to be the boss. This other cell fires before the SA node can.
What type of lifestyle changes should I teach a pt. With frequent PAC’c?
- Stress reduction techniques
- Limit caffeine
- Limit alcohol
- Exercise regularly
- Get adequate sleep
What are some common treatments for PAC’s?
- Infrequent PAC’s = Monitor pt.
- Increased frequency of PAC’s = AMIODERONE
- Count number of PAC’s
- Name underlying rhythm
What are some common causes of PAC’s?
- SNS Stimulation
- Anxiety
- Hypoxia
- Ischemia
What can increase PAC frequency?
- Anasthetics & Amphetamines
- Caffeine & Catecholamines
- Marijuana & Nicotine
- Hyperthyroidism.
