Person picking up medication scenario:
- Introduce yourself

- REVIEW MEDICATION PROFILE*Whenever someone wants a med filled, go through his or her whole profile
- Ask if they’ve taken this before

- Ask if there are any other concerns or questions
Smoking cessation scenario:
- Introduce yourself
- Ask how can I help you?
- Ask name and age
- any medical conditions?
MAPL
- how long have you been a smoker?
- how much do you smoke a day?
- ask if they’ve ever tried to quit
- why they’re trying to quit
- ask about lifestyle: alcohol/diet/exercise
- importance of quitting: be sure they know the benefits of quitting which are: reduced risk of all cancer, heart disease, and respiratory problems, food tastes better, save money, increased energy
- medical conditions/medications

- talk to them about smoking cessation products, help lines, keeping a diary of smoking, family and friend supports

Weight loss scenario:
- Introduce yourself
- Ask how can I help you?
- Ask name and age
- any medical conditions?
MAPL
-tell them it’s very good that they’re planning to lose weight because it’s very beneficial such as: reduced risk of heart disease, hypertension, stroke, it will increase self-esteem and decrease risk of depression
- ask about lifestyle (diet/exercise)
- smoking?
- alcohol?
- life style changes/stress?
- offer resources (such as Canada’s Food Guide), there are so many resources now a days online that can can help you prepare easy and healthy meals
- as for exercise, it’s really just about making it a regular part of your lifestyle.

Dry skin scenario:
- Introduce yourself
- Ask how can I help you?
- Ask name and age
- any medical conditions?
MAPL
- ask where/how big the area is?
- how long have they had dry skin
- any changes in life? - new house, new job, new laundry soap, etc.
- showering habits?
- laundry soap?
- perfume?
- water and caffeine intake?
- is it painful or itchy? Be sure to tell them that scratching increases risk of infection
- does it keep you up at night?
- have you had this before?
- have you tried anything to treat it before?
-allergies?
- interested in a cream, ointment or lotion?
- ointment: thick, heavily oil based, very greasy, but keeps moisture in, only have to apply it once or twice a day
- cream: not as thick, still beneficial to treat dry skin as it does keep water in as well. not as greasy
- lotion: heavily water based, have to reapply lots
If they have diabetes, REFER! Especially on feet/lower legs.
Bug bite scenario:
- Introduce yourself
- Ask how can I help you?
- Ask name and age
- any medical conditions?
MAPL
-determine what bit them: tick, mosquito, bees, etc.
-how long ago did the bite happen?
-where is the bite/how big?
-history of previous stings?
-is the bite painful and itchy?
-pain anywhere else in the body? like headache or body ache?
-fever?
TALK ABOUT SOLUTION:
-remove the insect part, ice the area, take an over the counter antihistamine for itching, try not to scratch and open the wound because that will just increase risk of infection
-monitor daily for signs of infection
-with mosquitoes you want to be concerned about west nile virus
MILD SYMPTOMS OF WEST NILE: flu like, body aches, head ache
SEVERE SYMPTOMS: confusion, muscle weakness, nausea, vomitting, stiff neck
-with ticks you want to be concerned about lyme disease: (bulls eye rash) or rocky mountain spotted fever: (rash, anorexia, nausea)
TALK ABOUT PREVENTION:
- long pants
- stay away from areas with lots of bugs
- limit time outside
- dont wear perfumes
- dont leave food outside
- avoid outside at dawn and dusk
- clean home and pets for fleas and bed bugs
- BUG SPRAY
DEET or ICARDIN: -do not use on babies under 6 months -look at percentages for when to apply: DEET: 6 months - 2 years: 10% 1D 2 years - 12 years: 10% TID 12+ - 30% TID ICARDIN: -a little bit stronger but same schedule except 20% has been approved not 30%
WHEN TO REFER:
- allergic rxn within 30 min
- history of previous stings
- under 2 yrs old
- stings to mouth and tongue
- unresolved prob after 7 days
- area worsens in 24 hours
Burn scenario:
- Introduce yourself

- any medical conditions?
MAPL
-ask where the burn is, how big it is, and how long you’ve had it
-determine if it’s 1st, 2nd(sup) or 2nd(deep) or 3rd
-IF 3RD DEGREE - REFER
-determine what caused the burn: (thermal, radiation, chemical, electrical)
IF ELECTRICAL - REFER!
if thermal or radiation talk about solution
IF CHEMICAL - be sure to remove all of the causative agent from the site of burn
SOLTUION:
- cool area with tepid water (not cold)
- do not pop blisters!
- make sure area is clean
- apply a non-stick sterile gauze
- can take acetaminophen for pain
WHEN TO REFER:
- burns where the skin is thinner - eyelids, inner arm, groin, face, ears
- children under 2
- adults over 55
- chemical, electrical or inhalation burns
- burns with more than a few blisters/large blisters or large areas of blistering
- medical conditions such as (diabetes, immunosuppressed, HIV)
DISCUSS PREVENTION:
- be safe
- SUNSCREEN IF ITS A RADIATION BURN
Sunscreen scenario:
- Introduce yourself

- any medical conditions?
- ask about family history of sunburns?
- ask about work/working conditions
MAPL
- skin type?
- waterproof?
- sweat proof?
- CDA approved!!
- lotions work better than sprays
- broad spectrum which covers both UVB and UVA rays
NON-PHARM PREVENTION:
- sunglasses
- hat
- long clothes
- SUNSCREEN
Offer them one and tell them how to apply:
- WEAR LIP BALM
- 15 to 30 mins before sun exposure
- REAPPLY
Tell them when to reapply:
- every 2 hours
- 40-80 mins after swimming or sweating
IF THEY GET A SUNBURN:
- stay out of the sun
- cool the area
- cover the area
- drink lots of water
- moisturize bc it will be dry
- if they have diabetes, a sunburn on the feet could lead to dry skin which may be concerning

- ask if they’re going on a trip
- ask who this is for (just themselves or a whole family?)
PREVENTION
- long pants
- stay away from areas with lots of bugs
- limit time outside

HCP scenario:

SBAR: Situation -ASK TO SPEAK WITH DR. \_\_\_\_ -introduce yourself, say you’re the pharmacist -severity of the problem -say how long the interaction will take Background -patient name/age, DOB, PHIN number, provide info about the situation -why the patient is here Assessment -what you think is going on -clinical information directly related to the problem: -current medications -allergies -adverse drug reactions -vital signs/lab results -recent changes related to concern Recommendation -plan of action is in order to resolve BE SURE TO CLARIFY AND THANK THE DOCTOR
Healthy food scenario (nutrition):

- very good that you’re interested in wanting to eat healthy because there are many benefits such as: decrease obesity, reduce risk of osteoporosis, increased energy levels, increased self-esteem, better digestion
- ask what their diet is like now?
- give them healthy food options: foods lower in fat, sugar, and salt, alcohol in moderation, follow canada’s food guide.
- lowering salty and fatty foods decreases risk of hypertension, cardiovascular disease and obesity
- lowering sugar foods decreases diabetes, increases oral health, and decreases obesity.
Patient Profile Scenario

CONFIRM EVERYTHING ON THEIR PATIENT PROFILE:
- name
- age
- adress
- allergies
- medical conditions
- ask what they want filled
- determine if that’s the only thing they have filled
- be aware of refills!!!
- ask why they aren’t filling something that they should be?
- if they forget - suggest an alarm clock or a rosette (pill calendar)
- if it’s too expensive - talk about pharmacare
- tell them that you will fill it for them right away
- “I’ll get this ready for you right away”
- ask if they have any other questions or concerns
