what are the benign neoplasms of the breast?
fibroadenoma, phyllodes tumor, intraductal papilloma, lobular carcinoma in situ (noninvasive proliferative lesion)
fibroadenoma vs phyllodes in define, epidemiology, clinical features, diagnostics and treatment?
fibroadenoma:
*common in 20-30yr old
*present with small painless, mobile (rubbery) mass
*on US/mammo: popcorn-like calcifications and pathology shows fibrous and glandular tissue
*tx: expectant or surgical excision
phyllodes tumor:
*common in 40-50yrs old
*present as large, painless, multinodular lump
*core needle will show leaf-like structure
*tx with surgical excision
intraductal papilloma vs lobular carcinoma in situ define, epidemiology, symptoms, diagnostic, tx?
intraductal papilloma:
*arise from epithelium of lactiferous duct
*typically presents as solitary mass behind the nipple and MOST COMMON cause of bloody nipple discharge (or serous)
*on US/mammo: mass within lactiferous duct
*tx: surveillance/surgical excision depending on the case
lobular carcinoma in situ:
*common in PREMENOPAUSAL women
*it’s a risk factor for malignancy
*ASYMPTOMATIC, INCIDENTAL finding in biopsy
*expectant management or surgical removal in select cases with risk of cancer
what are the cystic diseases?
fibrocystic changes, breast cyst, galactocele
fibrocystic change epidemiology, symptoms, treatment?
fibrocystic change:
most common benign condition
*CYCLIC premenstrual breast tenderness with multiple nodules BILATERALLY
*tx: symptomatic management with analgesics
breast cyst vs galactocele epidemiology, symptoms, diagnostic only for galactocele, tx?
breast cyst:
*single or multiple nodules variable in size may be tender and mobile
*asymptomatic= no intervention
galactocele:
*common in lactating women
*PAINLESS, subareolar mass (pain=infected)
*US/mammo= cyst with fat fluid level, fine needle biopsy=milk
*resolve spontaneously, needle aspiration for symptomatic relief
