defn of overdenture

A

a prosthesis that derives support from one/ more abutment teeth by completely enclosing them beneath its impression surface

reasons for overdenture

A

1) maintenance of alveolar bone
- allow for more stable and retentive dentures when there is more bone

2) sensory feedback
- tactile sensitivity discrimination
- teeth can detect minimum loads, assist in controlling masticatory force

3) reduction of psychological trauma
- prevents feeling of total loss and patient may be better able to adjust mentally for a transition to full dentures

indications for overdenture

A

1) single complete denture
- there will be greater masticatory forces than a complete denture in both jaws so we want the teeth to support

2) potentially unfavourable complete denture especially mandibular

3) when the tooth has extreme wear, not suitable for crowning

4) retain teeth as abutments to avoid free end saddle

disadvantages of overdenture

A

1) OH
- prone to caries and perio
- requires stringent OH

2) might be higher cost
- if copings are required when shape of root does not allow dome shaped prep or abutment is opposed by natrual teeth in patient with occlusal parafunction and just need to take more load
