ovarian hyperstimulation syndrome Flashcards
most common drug used to stimulate follicles
clomiphene citrate
what levels of estradiol are considered high
> 3500 can lead to ovarian hyperstimulation syndrome
what is the main factor that leads to vascular endothelial growth factor which then leads to ovarian hyperstimulation syndrome
HCG trigger
mathur classification of ovarian hyperstimulation syndrome
mild - ovarian size <8cm , no ascites
moderate- 8-12 cm, ascites can be seen in ultrasound
severe->12cm , clinical ascitis
critical , critical ascitis(respiratory distress near death)
in severe ohss hematocrit >45
in criticalohss hematocrit >55
management of ohss
- avoid pregnancy in severe and critical forms
- give oral fluids for mild and moderate forms
- remove fluids (tap ascites and effusion)
- iv fluids in severe and critical forms
drug of choice in ovarian hyperstimulation syndrome
albumin(colloid) it’s a big molecule
drug cabergoline
- its a dopamine agonist which inhibits VEGF(vascular endothelial growth factor) , indicated in ohss
- its given on HCG trigger day
- dose 0.5mg(8days)
