otitis externa Flashcards
List 4 important predisposing factors in otitis
- Conformation: hairy/stenotic canals, pendulous pinnae
- XS wetting of the ear canal: heat/humidity factors, swimming
- Obstructive ear disease: neoplasia, polyps
- Tx effects: inappropriate cleaning solutions, traumatic cleaning + plucking
What type of ear cleaners should you avoid in sensitive ears?
avoid acidic potent ceruminolytics and astringent ear cleaners
What are your 5 primary causes of otitis externa?
- Allergy: atopy, food, contact
- Endocrine: HypoT
- Autoimmune/immune-mediated
- Keratinisation disorders
- Ectoparasites: Otodectes cynotis, Demodex sp.
What conditions may benefit from ear drops containing glucocorticoids?
Allergic, immune-mediated primary causes
Give an example of a potent topical glucocorticoid
Mometasone, hydrocortisone aceponate
Give an example of a potent ceruminolytic
carbamide peroxide, sodium docusate, squalene
Give an example of a miticidal potentially found in ear drops
avermectins, permethrin, thiabendazole, rotenone
How may your cleaning solution differ when treating a dog with sebaceous adenitis vs. primary idiopathic seborrhoae?
- SA = dry ears –> tx/ w/ oil based
2. PIS = needs a potent ceruminolytic
Give 2 examples of immune-mediated primary causes of otitis externa
Pemphigus foliaceus, bullous pemphigoid
Give a gram +ve and gram -ve bacteria that act as secondary causes of otitis externa
Gram +ve: Staph, strep, enterococcus, corynebacterium
Gram -ve: Pseudomonas
List 2 perpetuating factors in otitis
- Pathological changes in the external ear canal
2. Otitis media
What is fusidic acid?
a narrow spectrum bacteriostatic antimicrobial
Fusidic acid targets…
Staph (+ methicillin-resistant strains), Corynebacterium, Anaerobes
List 3 aminoglycosides
Neomycin, Gentamicin, Framycetin, Amikacin
ID two disadv. of Neomycin (vs. other aminoglycosides)
- Least potent
2. Common cause of contact allergy
