heartburn > 3mo sever nocturnal alarm symptoms present wheezing/coughing nausea/vomiting chest pain with sweating
exclusions for self treatment meds
-recurring heartburn while taking recommended OTC or Rx h2 block or ppi
xclusions for self treatment groups
pregnancy and nursing mothers
children younger than 2 years for tums, 12 years for H2 block, and 18 years for ppis
adults > 45 years with new onset dyspepsia
Antacids
neutralize
dont alter secretions
relief 20-30min
side effects belching flatulence constipation diarrhea
H2 receptor antagonist
reduce acid secretion
onset is 30-45 minutes for 4-10 hours
side effects: fatigue, dizziness, const, diarrhea
tolerance develops with daily use
PPIs
reduce acid secretion 2-3 hour onset 12-24 hour duration relief for 1-4 days use for heartburn occurring at least 2days/wk same side effects as H2 blockers
concerns with PPIs and risks of long term use
- kidney disease
- dementia
- bone fracture
- heart issues
- infections
- micronutrient deficiencies
Treatment with OTC for HB
- limit use to 14 days
- regimen can be repeated after 4 months if needed
Diarrhea
increased frequency and liquidity of fecal discharge
- causes infectious or not
- can lead to dehydration and electrolyte abnormalities
Diarrhea prevention
hygiene
beware when traveling
diarrhea treatment
prevent dehydration
medication
if it lasts more than 48 hours get help

< 6 mo of age SEVERe Dehydration high fever blood mucus pus in stool vomiting severe abdominal pain pregnancy
