osteoarthritis Flashcards
what is the most common type of arthritis?
OA (3/4 cases)
what type of OA is most likely to cause disability?
knee OA, hand OA most common disease in elderly hand function
what comorbidities can complicate OA?
diabetes, heart disease, # limbs involved
what 3 factors are involved in the pathogenesis of OA?
biomechanical factors, proinflammatory mediators, proteases
how is inflammatory arthritis defined?
> 2000 leukocytes (cellular inflammation)
what systemic factor leads to OA?
altered joint loading (include injuries and malalignment, ACL tear always leads to OA)
what systemic factors contribute to OA?
female, genetics, obese (all leads to inflammation)
what is the role of hyaluronic acid in joint function?
provides viscosity and protects joint surface including chondrocyte zone from shear stresses
what degenerative changes happen in OA due to altered joint stress and inflammatory process?
- MMPs degrade proteoglycans
- collagen structure altered
- MMP13 collagenase breaks down collagen
- chondrocytes unable to repair
is OA an inflammatory arthritis?
no
what joints are commonly involved in OA?
DIP and PIP
back, knees, big toe
what factors are required to Dx OA knee?
+45, activity related joint pain, and morning stiffness <30min
is XR needed to Dx OA knee?
no
what are examples of non-pharm management for OA?
wt management, physical activity, assistive devices, GLA:D
what is the first line pharm tx for OA?
acetaminophen (but apparently doesn’t work)
