What is osteoarthritis
A mechanically driven disease of the synovial joints
It is a group of diseases where there is an imbalances between cartilage synthesis and degradation
How is a diagnosis of osteoarthritis made
Presence of pain plus typical changes on a radiograph
List some clinical features of osteoarthritis
Painful joints
Involvement of hands, knees and hips
List some o the symptoms of osteoarthritis
- Localised pain which increases on weight bearing
- Short lived or absent early morning joint stiffness
- No symptoms
List some of the signs of osteoarthritis
- On the hands outgrowths known as nodes may be seen on the fingers eg Heberden’s nodes or Bouchard’s nodes
- Generalised muscle wasting
- Radiographic signs
What are Bouchard’s nodes
Proximal interphalangeal nodes (PIP)
What are Heberden’s nodes
Distal interphalangeal nodes (DIP) found towards the ends of the fingers
Give some radiographic signs of osteoarthritis
- Joint space narrowing
- Osteophytes
- Sclerosis
- Subchondral bone cysts
What are some of the risk factors for primary osteoarthritis
- Incidence and prevalence increases with age
- more common in women
- More common in populations where load on joints is greater eg obesity or runners
What are some of the risk factors for secondary osteoarthritis
- More predictable following severity in the damaged joints
How do we manage osteoarthritis
- Pharmacological methods
- Non pharmacological methods
List some pharmacological management for osteoarthritis
- Analgesics particularly NSAIDs
- Injections of steroids/ hyaluronan
List some non pharmacological management’s for osteoarthritis
- Exercises
- Weight loss
- Education and support
