Osseointegration Flashcards

1
Q

What happens to the ridge after extraction?

A

It will change shape.

Mean loss of 50% of bone height.

Q

What happens to soft tissue after extraction?

A

Extraction leads to narrowing of the band pf attached gingival mucosa and effectively coronal shifting of the mucosal-gingival junction.

Q

Why should we wait a while after extraction before placing an implant?

A

The bone remodels so we need to wait until the bone shape has stabilized.

Q

What happens to the buccal bone plate if it is thin during an extraction?

A

There is a collapse in the alveolar ridge and buccal bone thinning.

Q

How can we tell someones had an implant by looking at the gingiva?

A

The alveolar ridge has collapsed in.

Q

Does an implant reduce the rate of absorption?

A

NO!!

Therefore, to place an implant we need to have bone all the way around the implant.

Q

If an implant needs to be placed immediately, where do we position it?

Q

What is the problem with placing an implant early?

A

The greater the risk of bone loss around the implant.

9
Q

How do we position the implant during placement?

A

Early implants need to be placed 2-3mm of buccal bone over implant.

They should not be placed axially in the socket and not filling the whole socket.

Q

What can bone grafting do when placing an implant?

A

To avoid this, we can do one grafting or bone substitute process to place an implant more bucally.

We could also do a soft tissue graft to thicken the soft tissues to hide the degree of buccal bone collapse.

Q

What is osseointegration?

A

A direct functional and structural connection between living bone and the surface of the implant.

It is dependant on good primary stability at the implant placement so the implant cannot move within the site during healing.

Q

What is the process of ossseointegration?

A

1) Bleeding around the area, coagulum formation (coagulation of blood)
2) Ingrowth of blood vessels
3) Deposition of osteoid
4) Provisional matrix
5) Formation of irregular woven bone
6) Maturation into lamellar bone

Q

How does modifying the implant surface help osseointegration?

A

Roughened surface by balloting with particles or using etching to accelerate the integration process.

Q

What is contact osteogenesis that occurs on implants?

A

This means there is bone formation directly on the implant surface rather than on a flat surface where the bone is forming from the wound towards the implant.

Q

What happens in a site after extraction?

A
  • Initial blood clot and fibrin replaced by immature vascularised connective tissue
  • Vascular structures form
  • Woven bone forms in area
  • Woven bone replaced with lamellae bone
