What is Osgood-Schlatter Disease?
Inflammation at the tibial tuberosity where the patella ligament inserts causing anterior unilateral knee pain
Who is typically affected by OSD?
10-15 year olds
Males
What is the pathophysiology of OSD?
Multiple minor avulsion fractures occur where the patella ligament pulls away pieces of the bone
This causes growth of the tibial tuberosity causing a visible lump below the knee
What happens to the lump below the knee in OSD?
Initially tender due to inflammation
As the bone heals the lump becomes hard and non-tender
Lump is then permanently present at the tibial tuberosity
What is the presentation of OSD?
Presents with a gradual onset of symptoms
Visible or palpable hard and tender lump at tibial tuberosity
Pain in anterior knee
Pain exacerbated by physical activity
How is Osgood-Schlatter Disease managed?
Reduction in physical activity
ICE
NSAIDs
After symptoms are settled, physiotherapy to strengthen joint and improve function
What is the prognosis of Osgood Schlatter Disease?
Symptoms fully resolve over time
Patient left with a hard bony lump (usually)
Rarely a complete avulsion fracture, tibial tuberosity separated from rest of the tibia
