• Gentamicin or tobramycin, 3 milligrams/kg/d divided every 8 h, ± ampicillin, 2 grams
every 6 h
• Piperacillin-tazobactam, 3.375 grams IV every 6 h
• Cefepime, 1–2 grams IV every 8 h
• Ertapenem, 1 gram IV every day
• Imipenem, 500 milligrams IV every 8 h
• Meropenem, 1 gram IV every 8 h
