Orthostatic Hypotension Flashcards
1
Q
What is orthostatic hypotension?
A
-20 mm decrease in systolic
-10 mm decrease in diastolic
w/in 3 min of standing AFTER lying down for 5 min
2
Q
What is it commonly associated with?
A
- autonomic failure
- volume depletion
3
Q
What is the biggest RF?
A
- age (they are dehydrated and they don’t know in the elderly)
- standing all day at work
- removal of colon b/c nothing is being absorbed
4
Q
What is the pathophysiology?
A
standing causes 500-1000ml pools the feet –> decreases venous return to the heart –> decreases CO –> baroreceptors and aortic arch provoke reflex response –> increases sympathetic flow –> increases peripheral resistance to increase venous return
5
Q
Sx of volume depletion?
A
- D&V
- hyperglycemia
- hemorrhage
- fever
6
Q
Sx?
A
Head: -headache -lightheadedness -syncope Eyes: -blurred vision/darkening of fields Body: -weakness -nausea -pallor -diaphoresis -palpitation
7
Q
PE?
A
check for signs of O2 and perfusion
8
Q
ECG?
A
ONLY when you CLEARLY know the cx
9
Q
Tx?
A
Chronic condition: ANS -rise slowly -elevate head of bed -avoid exertion -elastic stockings Volume depletion -IV
