Rust’s Sign
Pt. stabalizing head/neck. If present, suggests a severe upper cervical atlanto-axial instability. X-ray before any other tests
Libman’s Sign
Thumb pressure over mastoid process grad. inc. until pt. states uncomfortable. assesses pain threshold for palpation findings of later exams
Bakody Sign
Pt. puts arm on top of head. If pain dec. + Bakody sign. Note location of pain to ID etiology
Reverse Bakody
Pt. pain inc. indicates TOS due to interscalene compression.
Bikele’s Sign
Pt. arm extended behind them stressing the brachial plexus. + pain will radiate, note location
Brachial Plexus Tension Test
Pt. hands on back of head, hip into pt. back and pull elbows back. + will have radicular pain, note location
Dejerine’s Sign
Coughing, sneezing, and straining during defecation may cause radicular symptoms indicating a SOL at the C(?) level. Herniated or protruding IVD, spinal cord tumor, compression fx
Valsalva’s Maneuver
Pt. takes breath and bears down. tests for SOL at the level of pain
Swallowing Test
Pt. instructed to swallow. Pain or difficulty indicates dysphagia is present suggesting esophageal irritation due to direct trauma or retroesophageal SOL
Naffziger’s Test (Not on a high risk cardiac patient)
Dr. behind pt. occludes external jugular veins for 10-15 sec then asks pt. to cough. + sharp pain at level of lesion
Barr’e-Lie’ou Test
Pt. seated rotates head back and forth as fast as they can. + if pain at any time. rules our vascular insufficiency, cervicogenic vertigo, and possible vestibular apparatus abnormality

Distraction Test
Dr. distracts pt. head. Generalized inc. in pain indicates m. spasm. dec. indicates IVF encroachment or facet capsulitis + relieves radicular symptoms
