a. A description of the applicant organization, including:
- History: Provide a brief description of the history of the organization in 300 words or fewer.
- Demographics of the population served.
- Demographics of the registered nurse (RN) staff.
- A link to the external webpage where the general public learns about the contributions of professional nursing in your organization.

a. 2 copies of the most recent:
- Reports prepared for the organization’s stakeholders (include nursing and organizational reports; they may be combined
- Quality plans
- Patient safety plan
- Strategic plans for the organization and nursing services (include nursing and organizational plans; these may be combined)

a. Provide:
- The current chief nursing officer’s (CNO) job description.
- CNO’s curriculum vitae or resume.
- The organizational chart(s).
- The nursing organizational chart(s).
- A description of the CNO’s structural and operational relationships to all areas in which nursing is practiced.
- Name and credentials, of all individuals serving as CNO, in the last 48-month preceding document submission. Include all the tenure dates.

a. The policies and procedures that govern and guide continuing professional development programs.

a. Provide the most recent:
- All RNs who influence or provide care.
- All settings where RN practice (e.g., inpatient care, ambulatory, and medical practices).

a. Provide a description of the process by which the CNO (or designee) partcipates in:
- Credentialing, privileging, and evaluating of all advanced practiced registered nurses (APRNs).
- Frequency of reprivileging.

a. Provide the policies or equivalent evidence of, the organization’s workplace advocacy initiatives for all staff, including the following issues:
- Caregiver stress
- Diversity
- Rights
- Confidentiality
- Care for the impaired practitioner

a. Provide the policy or equivalent evidence, which permit and encourage nurses to:
- Confidentially express concerns regarding their professional practice environment, whithout retribution.
- Address the identification and management of problems related to incompetent, unsafe, or unprofessional practice or conduct.
- Address interprofesional conflict.

a. Provide the policies, procedures, charters, attestation, or bylaws (including institutional review board) that:
- Protect the rights of participants in human subject research.
- Designates at least one nurse must be a voting member of the governing body responsable for the protection of human research participants.
OR
b. Provide a letter signed and dated by the chair of the governing body responsable for the protection of human research participants that attests to this requirement.
- If providing an attestation of “at least one nurse is a voting member on the governing body,” the template provided on the website is required.

a. Provide table of nursing research studies (ongoing and completed) within the most recent forty-eight months.
