Mechanism: Orbital Blow Out Fracture
Small diameter object strikes globe
Fracture of the inferior or medial orbit walls
No orbital ridge or rim fracture
Herniation or entrapment of adipose, inferior rectus or the inferior oblique within the maxillary or ethmoid sinus
Clinical Features: Orbital Blow Out
Enophthalmos (herniation of globe contents)
Infraorbital anesthesia
Diplopia with upward gaze (entrapment of IR, IO, orbital fat)
Clinical Features: Naso-orbito-ethmoid fracture
Pain with eye movement
Traumatic telecanthus
Epiphora
CSF leak
ED Management: Orbital Blowout Fracture
Clavulin 875 mg
1 tab po bid
Decongestants:
Phenylephrine Intranasal spray
2-3 spray per nostril q 4 hrs
No nose blowing
Follow up with plastic surgery
