Types of estrogen used
17-beta estradiol (preferred) or conjugated equine estrogens (CEE)
Ultra low dose
0.25mg/day
Standard dose/moderate HF
0.5mg/day
High dose/severe HF
1mg po daily
Preferred type of progestins used
Oral natural micronized progesterone
Standard progestin dose (daily or cyclic)
100 mg daily or 200mg/day of 12 days/month
Other types of oral progestin
MPA 2.5mg daily - may be associated with a HR risk of BCA and CHD
Side effects of HT
Estrogen - breast tenderness; Progesterone - mood symptoms and bloating; Vaginal bleeding with cyclic E-P regimens
Alternatives to oral progestin
(1) Vaginal micronized progesterone (2) Levonorgestrel IUDs (3) Conjugated estrogen/bazedoxifene
