You are at the event of a very sick patient. The spouse demands that you immediately administer X treatment which is unknown to you. What do you do?
1-Remember this is a stressful and difficult time for her
2-The care we provide is dictated by the Dept and our Pt Care Protocols
3-Attempt to diffuse and re-direct
a.she has important medical history, meds and events leading up to today
4-She can help me get her husband to the hospital to get this important procedure
5- rare but SPD is an option
You arrive on scene and find a physician rendering aid to a pt. They want to maintain control over the scene. What do you do?
1-Must be from out of town
2-Medical control. We have control over the scene for resources, extrication or decon etc
3-The doc can keep control over the pt if they are willing to stay with them until the ER
a. Have them talk to the Med 1 doc
4-Support them within our protocols and ensure the best care for the pt
Why do you want to be a medic?
1-What I’ve wanted since my first exposure to EMS 20 years ago
2-Paramedic training is the single best training available in the fire service
a. Makes a real difference in Seattle by using their training to save lives.
3-Witnessed the teamwork
a. motivated by the leadership role that they serve
b. I request additional training and they always seem happy to oblige.
4- Integrity of Medic 1. The basic tenet that every pt from the homeless pt to the top official gets the same top notch service
5-Proud to be part of the premiere ALS service provide in the country if not the planet
6-I care so I want to provide a higher level of service because I want to help people
What are your qualifications to be a Medic?
1-Over 15 years experience, 14 years on busy companies a. E39 - packaged pt for Medic arrival b.served a diverse population c.E31-station with medics 2-Almost 20 years of training with SFD medics a.supplemental PHTLS, sick not sick b.trained AHA CPR for 5 years c.15 years of CBT d.5 Redmond Symposiums e. Tues Series 3-Eboard member a.committee/team work - personal dynamics 4-Written correspondence a.Office Suite b.Outlook and Sharepoint 5-Attitude - pleasant even during tough tasks 6-Personal - Elections a.Respectful/Honest/Work Hard-follow up b.Like to help because I care
Describe an incident where I did something wrong on a call. What did I do to fix it?
1-Pinehurst Pub Stabbing 2-Influences a.SPD b.Code Green c. Multiple pts 3-AMR run later with feedback later that night a. flank stabbing b.horrified c. immediate review 4-Discuss openly while fresh a. acknowledge mistakes w/o defensive b.discuss solutions and strategies 5-MIstakes a.SPD: Lt/Driver get witness statements ff's do exam b.Code Green too soon situational awareness handle our business/keep resources available communicate clearly w/SPD 6-Last mistake was an incomplete exam a.verbalize our findings to double check findings 7-Corrections were effective a.drilled the skills to reinforce the lessons b.better crew that delivered better care
What do you think will be stressful about medic training and how will you manage it?
1-Several things
a.maintain the prestige and honor of a premier program
b.pressure to perform at a high level
2-Higher level of care means more life impacting decisions on a daily basis
a.mistakes can be catastrophic
b.highly technical decisions with potential consequences
3-Physically stressful
a.busy companies mean less sleep
b.working in cramped spaces w/lifting and contorting
4-Emotional stress
a.communicate with family members after significant events
b.sharing pain can have cumulative effects
5-Rely on basic strategies to manage stress
a.good support network
-family/SFD family/friends and mentors
-I reach out to my friends when I think they need help and I’m not afraid to reach out when I need it
-EAP and SCN
b.Take care of the physical stuff
-diet/exercise/sleep
c.rely on training and experience w/fires because we drill it and I’ve seen it so I hope to develop that after some time as a medic
