Explain what Sulfonylureas do
They stimulate the release of insulin from the islet pancreatic beta cells
What dont we give sulfonylureas to patients with type 1?
Because the medication requires functioning beta cells
What’s the common adverse reaction of sulfonylureas?
Hypoglycemia
What is the Sulfonylurea of choice?
Glibenclamide
Glibenclamide dosage
2.5mg/day, upped to 5-10mg maintance dose daily
Why don’t we use Glibenclamide in patients of liver failure or CKD?
The risk of Hypoglycemia
Glipizide is given to the elderly in place of Glibenclamide, why?
Lower potency and shorter duration of action. Essentially weaker
We sometimes give Metformin and Thiazolidinedione, why?
As they don’t cause Hypoglycemia
What is the GLP-1 receptor agonist of choice?
Liraglutide
