When is a fracture open?
A
When there is direct communication between the fracture site and the external environment - most commonly the skin.

How does someone with an open fracture present?
A
Post trauma
Obvious deformity - wound or punctum (bone protruding)
Pain and swelling

What investigations are done for open fractures?
A
Bloods - clotting screen + group and save
Neurovascular examination
XRAY

How is an open fracture managed?
A
Fluid rescus
Analgesia
Broad spectrum abx. + tetanus vaccination
Urgent realingement and splinting
Surgery
- debridement of wound as needed
- wash out with saline
- skeletal stabilisation - e.g. screws
- plastic surgery may need for soft tissue / skin coverage
- ensure no vascular surgery is needed
