A
- Patient has a right to be informed of what is happening to them
- To minimise harm to the patient
- We have a duty of care to the paitnet
- To maintain trust in the dentist-patient relationship
- To gian informed consent for any further treatment related to the incident
- To prevent a recurrence of the incident to others
- To possibly avoid formal complaint

A
- An apology or expression of regret
- A factual explanation of what happened
- An opportunity for the patient to relate their experience
- An explanation of the steps being taken to manage the event and prevent recurrence
- Appropriate documentation of the process

A
Open disclosure is the process of providing an open, consistent approach to communicating with patients/consumers, their family, carer and/or support person following a patient incident. The process includes expressing regret or saying sorry.

A
- Acknowledgement of the incident, and offering initiating or signalling the need for open disclosure
- Preparing for, and engagin in open disclosure discussion, including expressing regret
- Providing follow up to patient and family/caregiver, including actions taken as a result of the investigation
- Completing process and maintaining documentation
