Oligohydramnios Flashcards
Amniotic fluid volume less than expected for the gestational age
Oligohydramnios
Measurements
AFI , amniotic fluid index <5cm
SDP, single deep pocket <2cm
Risk factors
Gestational age
37-42 wks term
>40 weeks
Pathogenesis
Decrease in fluid production in relation to fluid removal
Decreased amniotic fluid volume-> compressions
Pulmonary hypoplasia
Etiology leading to oligohydramnios
PROM
Placental insufficiency leading to IUGR, intrauterine growth restriction
Fetal anomalies
Medications affecting renal blood flow, ACE inhibitors, prostaglandin NSAIDs
Diagnosis
Small uterine size
Inconsistent fetal movement
PROM, leaking fluid
Ultrasound
MANAGEMENT
Antenatal management
Hydration
No effective long term treatment
Monitoring
Fetal kick counting
Non stress test
Biophysical profile
Delivery
Monitoring
Vaginal delivery
Amnioinfusion
Complications
First trimester
Spontaneous abortion
No treatment
Second trimester
Pulmonary hypoplasia
Elective termination
Third trimester
High survival rate
