Why are ACEi’s and ARBs a risk for AKI?

A

ACE inhibitors or ARBs generally preserve renal function. However, these medicines can also decrease glomerular filtration by causing vasodilation of the efferent renal arteriole. These drugs can lead to impaired renal autoregulation, thus lowering GFR. (hence build up of creatinine and less urine)

Whats polymyalgia rheumatica?

A

Is a clinical syndrome characterised by pain and stiffness of the shoulder, hip girdles and neck. (more proximal joints are affected!)

Usually occurs in elderly, new onset.

PMR has lack of swelling therefore not RA
