oesophageal cancer Flashcards
give 3 radical tx options for oesophageal cancer
oesophogectomy - removal of cancerous tissue from oesophagus
ChemoRAD - EBRT =
middle + lower 1/3) 5 field conformal, two post oblique, two ant oblique and one ant. wing board , knee rest
upper 1/3) 2 or 3 field conformal 2 ant oblique 30 degree wedged with one ant - thermoplastic shell to immobilise head and neck
chemo = 4 cycles of cisplatin and capecitabine
neoadjuatnt chemo = weekly placitaxel and carboplatin for 5 cycles concomitant with RT 45Gy in25#
how does oesophegectomy work
removal of cancers part of oesophagus and stomach/part of bowel is moved to replace part removed- for all stages as long as tumour resectable
4 RT specific challenges for oesophageal cancer
tumour volumes
anatomical situation
poor normal tissue tolerances
organ motion management
side effect of oesophageal cancer
acid reflux
weightloss
difficulty swallowing
erythema
fatigue
SCOPE-2 trial for oesophogeal cancer pts
improving chemorad by increasing RT dose and changing chemo drug to carboplatin and paclitaxel
