med history reviewed

do replacements have to have prophylaxis?
1) no longer recommended (by AHA and ADA)
2) any other meds for the replacement, any complications?
high blood pressure and diabetes?
1) what meds, is it under control?
2) Hba1c? <6.5% for diabetic is controlled
3) blood pressure 120/80 is controlled

1) pacmanual
what would you change about dental appt
1) make sure the position changes are slower due to orthostatic hypotension (medication)
2) see her in morning for diabetes
- when they take pills and check sugar
3) MAKE SURE THEY KNOW TO TAKE BREAKFAST
check radiographs updated
1) make sure they are updated
2) RADRPT FORM!!!

1) you have to present the patient to the faculty after pt approve TP and signs
2) present their CC
step 1
1) history, vitals, EOE, IOE
- COHRI form
- CRA form
- ODH&E
2) ask more questions
- gather dental and social histories
3) BP, pulse, resp
- you can take BG reading if applicable
4) health history risk assessment

- and take photos if needed, or impressions if needed for TP options

- baseline clinic eval or PEBASE
2) OHI

1) HTE
- odontogram
2) RSIOE form
- TMJ screening
3) complete CRA form
take photos and impressions if not done already
PRESENT TO FACULTY thoughts on findings and tx options
step 4
1)ideal TP and alternate TPs
- Urgent, disease control, reconstructive, maintenance
2) discuss plans and alternatives with faculty and patient
3) sequence according to patients chief concerns
4) get approval and signature
5) financial approval
6) SOAP note
