  • phytomenadione (vitamin K)
  • given by slow IV injection
  • INR 5-8 & no bleeding: WH 1 or 2 doses, reduce subsequent maintenance dose
  • INR 5-8 & minor bleeding: phytomenadione, stop warfarin, restart when INR <5
