- Facial palsy
- Skull fracture
- Intracranial haemorrhage
- Face abrasions

Indications of instrumental delivery
A
- Emergency situation (e.g. fetal distress)
- Inadequate maternal pushing effort → prolonged 2nd stage of labour
- Maternal effort is prohibited (e.g. cardiovascular diseases)

Pre-operative assessment and preparation for instrumental delivery
A
- Hx review
- Abdominal exam: fetal head not palpable
- PV exam:
- Cervical dilatation
- Fetal head station
- Fetal head position
- Fetal head asynclitism
- Fetal head moulding & caput
- Maternal pelvis (for any contracted pelvis)
- Preparation:
CACACL
- Consent
- Aseptic technique
- Catheterize urinary bladder then remove Foley
- Pudendal anaesthesia
- Choose appropriate forceps / vacuum cups & check if they are lockable
- Lubricate the forceps / vacuum cups
