Obesity 3: obesity and its management Flashcards
What is the effect of liposuction on CV risk
No difference as it only affects subcutaneous fat not visceral fat
What is surgical omentectomy
Gastric banding and/or mental removal
Effect of weight loss on visceral fat
Significant improvements in CAD risk profile. There is a large loss in visceral fat
Effects if leptin therapy
- Decreased appetite
- No increase in BMR
- No adverse effects
- Development of non-neutralising antibodies
How does the number of copies of the FTO gene variant correlate with extra weight
Increase in number of copies of FTO gene variant, the more the extra weight
What are the common genetic causes of obesity
FTO gene
POMC mutation
What does cutting calories put you into
Starvation mode
What is the key to losing weight in the long term
Maintaining basal metabolism or keeping ‘calories out’ high
What is a reasonable goal for weight loss
8-10% reduction in weight in first 6 months. most weight loss in first 12 weeks
What is the treatment of obesity
After dietary and lifestyle modifications
- IN combination with other treatments:
- Orlistat
- Saxenda
- Sibutramine
- Rimonabant (not yet released)
What is the mode of action of sibutramine
Centrally acting dopamine, serotonin, and noradrenergic reuptake inhibitor.
Also stimulates thermogenesis which increases energy use
What are cautions for sibutramine and how to overcome
Hypertension and vascular disease. Monitor BP and heart rate
What is the mode of action of rimonabant
CB1 receptor blocker (are one of two receptors in the endocannabinoid system)
-The drug eliminates endocannabinoid-induced hyperpahagia and fat accumulation and reduces motivation for nicotine administration
What are bariatric surgery procedures which can help manage obesity
Gastric band- restriction
Sleeve gastrectomy- restriction
Gastric bypass-restriction and malabsorption
