Things to look out for directly following birth

A

-If skin is yellow, put under UV light to turn unconjugated bilirubin into conjugated
-Feet may be blue
-Vaginal bleeding normal in females (false menses)
-Steroid moist gauze on spina bifida
-Head elongated for 1st few days to allow easier movement through vaginal canal
-Hematoma on head will go away on own
-Vernix caseosa is cheesy substance on body that protects skin from amniotic fluid
-Lanugo is fine, soft hair on body
-Count apical HR for 1 full min
-1st bath after temperature is stable
-Clicking sound when auscultating lungs is normal
-Breastfeed baby for 10 min before switching breasts
-7 mL of milk for 1st 24 hours, can drink more w/ time

Stages of labor

A

-1st: Cervix dilates from 0-10 cm
-2nd: Baby is out of vagina
-3rd: Expulsion of placenta (dull side = mother’s, shiny side = baby’s)
-4th: Resting, assessing for bleeding

BUBBLE assessment

A

-Postpartum assessment
-Breast: milk necessary for life of newborn, don’t want nipple to be too engorged, can lead to mastitis, keep breast clean
-Uterus: auscultate abdomen 1st, tell mother to gently massage where uterus is to make it go back down into pelvis, can be slightly above or below umbilicus following birth, won’t feel it anymore after 9-10 days
-Bladder: ask if have been urinating, measure 1st and 2nd urine sample within 8 hr period, eat extra 300 calories if breastfeeding, breastfeeding will make mothers lose weight
-Bowel: ask if passing gas or have been voiding
-Lochia: vaginal discharge, mix of blood, mucus, and uterine tissue, (rubra: dark red for 3-4 days, serosa: pinkish brown for 4-10 days, alba: whitish yellow for 10-28 days)
-Episiotomy: surgical incision made between vagina and anus to allow for passage of baby
