Short bowel syndrome
2 metres or less from duodenojejunal flexure
Critical lengths
Less than 100cm jejunum - long term IV fluid and electrolyte
Less than 75cm jejunum - long term PN, fluid and electrolyte
Less than 50cm jejunum + colon - long term PN, fluid and electrolyte
Oral fluid to prevent dehydration and electrolyte imbalance
Decrease oral fluids
Hypotonic fluids of less than 90 mmol sodium pull sodium and hence water into lumen leading to high stoma output
Hypertonic fluid induce secretion from enterocytes to dilute luminal content which also leads to high stoma output
High fat diet for no colon
Hyperphagic diet
Urinary sodium value that indicates dehydration
Less than 20mmol/L
