Describe the NCPM. [4]
- Standardized process
- Emphasizes relationship between patient/client and nutrition professionals
- Uses current scientific evidence to inform practice in meeting individualized needs
- Most commonly used with individuals in a clinical setting
What is at the core of the NCPM?
- Relationship with client
Which systems are outside the NCPM? [2]
- Screening & referral: identify those who may benefit from nutrition care; refer a client to another professional
- Outcomes management: improve the NCP; informs research and further assessment (i.e., another round of ‘ADIME’)
What is the outermost ring of the NCPM?
Environmental factors: environment in which the practitioner operates; impacts the client’s ability to benefit from the NCPM
e.g., practice setting; health-care system; social system; economics
What is the innermost ring of the NCMP?
Capability of practitioner: what the professional brings to the client
e.g., collaborative practice; evidence-based; ethical; nutrition skills & competencies; clear communication; standardized documentation
Describe the ‘assessment’ portion of the NCMP.
- Use of appropriate methods & indicators
- Determines existence & severity of problem
- Forms foundation for reassessment in Monitoring & Evaluation
- Valid & reliable data
- Categorized (A, B, C, D, E)
- Integrated
- Leads to diagnosis
Describe the ‘diagnosis’ portion of the NCMP.
- Documented with a PES statement
- “Problem” related to “cause” as evidenced by “signs & symptoms
- NOT a medical diagnosis
PES = Problem, Etiology, Signs & Symptoms
What are the 3 domains of problems within diagnosis?
- Intake
- Clinical
- Behavioural/environmental
Differentiate between signs & symptoms.
Signs: objective data; observable & measurable
Symptoms: subjective data; what the client feels & expresses
Describe the ‘intervention’ portion of NCMP.
- Planning & implementing strategies to improve/resolve nutrition problems
Describe the ‘monitoring & evaluation’ portion of the NCMP.
- Objectively determine if SMART goals/outcomes were met
- Results loop back into assessment
