What are the five steps of the nursing process?
Assessment, Nursing Diagnosis, Planning, Implementation, Evaluation.
What is the purpose of the nursing process?
To plan and deliver organized, purposeful nursing care that makes the patient feel safe and secure.
What does assessment involve in the nursing process?
Collecting information about the person’s health history, family history, doctor’s information, test results, and medical records.
What is the difference between objective and subjective data?
Objective data are observable signs; subjective data are symptoms reported by the patient.
What is a nursing diagnosis?
A health problem identified from assessment information that can be treated by nursing measures.
What is the goal of the planning step in the nursing process?
To set priorities and goals aimed at the person’s highest level of well-being and function.
What does a nursing care plan include?
Nursing diagnoses, goals, and measures for each goal.
What happens during the implementation step?
Nursing measures from the care plan are performed, tasks are delegated, and care is reported and recorded.
What is evaluated during the evaluation step?
Whether the goals in the planning step have been met, and any necessary changes to diagnoses, goals, or the care plan are made.
What role do nursing assistants play in the nursing process?
They make observations used for diagnoses and planning, help develop the care plan, and carry out tasks from the assignment sheet.
