What are some considerations when maintaining dental implants v natural teeth
There are no gingivodental or transseptal fibers
No PDL - osseointegration
Maintaining attachment is very important
Oral hygiene is very important
Lax oral hygiene effort can result in bacterial infection leading to gum disease, bone loss, and tooth/implant loss
Define perimucositis
Presence of inflammation much like what we see in gingivitis
Define periimplantitis
Inflammatory response around an implant that is comparable to periodontitis
Presents itself as a bowl-shaped loss of bone around the implant leading to loss of attachment
Implant is likely to fail
What are some mechanical complications
Broken parts
Fractured abutments and/or retainer screws
Poor oral hygiene
Poor tissue response
Understand instrumentation and chemotherapeutic agents used in treating patients with dental implants
Professional mechanical debridement using carbide or titanium scaler
An implant ultrasonic scaler with plastic tip (can also use gold)
Antimicrobial rinses
Local delivery antibiotics
Non-metal instruments are paramount, as scratches of the implant increases plaque formation
What are important things in providing the best care for implants?
They’re standard of care
In-depth planning is required
Longevity of the final restoration is dependent upon entire team
Dental hygienist is involved in all phases of treatment, and is often the one to deliver the earliest treatment
What occurs at maintenance visits?
Do probing and evaluation
Gentle probing with a presence of bleeding implies inflammation
Probing depths and bleeding indicate further examination is necessary
