Normocytic anaemia Flashcards
What is Normocytic anaemia?
anaemia with a normal MCV (80-100).
What are 7 causes of normocytic anaemia?
Acute blood loss Anaemia of chronic disease Bone marrow failure (Aplastic anaemia) Renal failure Hypothyroidism Haemolysis Pregnancy
List 4 signs of normocytic anaemia
Pallor
Hyperdynamic circulation: Tachycardia, ESM, Cardiac enlargement
List 7 symptoms of normocytic anaemia
Fatigue Dyspnoea Faintness Palpitations Headache Tinnitus Anorexia
What sign indicates haemolytic anaemia? What would indicate this on a blood test?
Jaundice
High unconjuated BR
What would be seen in the blood if ACD was the cause of normocytic anaemia?
Low serum iron
Low transferrin
Low/ Normal total iron binding capacity
High ferritin due to increased storage + retention within reticuloendothelial system + immune activation (ACP)
How is normocytic anaemia managed?
Treat underlying cause
What is the pathogenesis of anaemia of chronic disease?
Chronic disease causes inflammation + release of cytokines
Stimulates Hepcidin release which decreases Fe uptake, increases Fe storage + decreases Fe transport
What investigations are made for normocytic anaemia?
FBC: Hb, MCV, WCC, platelets
Check RDW
Check hx for haemorrhage
What does RDW indicate in normocytic anaemia?
Low/norm: ACD
High: haemolytic anaemia
What should be suspected if WCC or platelets are low in normocytic anaemia?
Marrow failure
