what key physiological changes must occur to allow for labour to start ?
cervix softens
myometrial tone changes to allow for contractions
progesterone decreases, oxytocin and prostaglandin increase to allow for labour to initiate
what is stage 1 of labour ?
complete when cervix is fully dilated
progress usually 0.5-1cm / hour
what is stage 2 of labour ?
from full cervical dilation to birth
can be active or passive
primigravida birth expected within 2 hours
multigravida expected within 1 hour
what is stage 3 of labour ?
time from birth expulsion of placenta
ative management
delayed is >30 mins of active management
how is labour monitored ?
maternal observation abdominal palpation vaginal examination auscultation of fetal heart CTG
what are the steps in mechanism of labour ?
descent flexion internal rotation of the head crowning and extension of the head restitution internal rotation of the shoulders external rotation of the head lateral flexion
what are analgesia options in labour ?
breathing, massage, DHC entonox - NO opioids - morphine remifentanil - patient controlled epidural
